Physician
Provide Advanced Primary Care to homebound patients with a multi-disciplinary team. Requires an M.D. or D.O. and licensure in Illinois, Indiana, or Florida.
Physician Assistant
Deliver home-based care with independence and support, including a company vehicle, equipment, and malpractice coverage. Requires a PA degree and state licensure.
Nurse Practitioner
Provide Advanced Primary Care to homebound and underserved patients, with flexible full-time and part-time scheduling. Requires NP licensure.
Podiatrist
Deliver hands-on podiatric care to homebound patients while enjoying the autonomy and flexibility of a home-based practice model.
Social Worker
Support the psycho-social needs of patients across the greater Chicagoland area as part of a multi-disciplinary care team.
