What is home-based primary care?
Home-based primary care brings a physician-led medical team into the patient's home, providing the same primary care services available in a traditional office, for patients who have difficulty leaving home.
Who qualifies for care?
We care for homebound and chronically ill patients who have difficulty leaving home due to mobility limitations, cognitive impairment, the need for special transportation, or a medical condition. If you are unsure, call us and we will help you find out.
What areas do you serve?
We provide in-home care across Northern Illinois, Northwest Indiana, and the greater Chicagoland area, and throughout South Florida, including Plantation and surrounding areas.
Do you accept insurance?
Yes. We accept Medicare and a range of Medicare Advantage and private insurance plans across both regions. See our Insurances Accepted page for the current list.
How do I schedule a visit?
Call the office for your region, or use the Schedule a Home Visit form on our site. Our team will follow up to arrange your visit.
What should I do in a medical emergency?
In a medical emergency, call 911 immediately. Our home visit service is not for emergency care.
